Hello and welcome. My name is James Titcombe, and I’ve been working in the area of patient safety for over 10 years now. My journey into patient safety work started in 2008 when sadly, my baby son died due to failures in care at the hospital where he was born.
This tragedy changed my life and in 2013, I took a decision to leave the nuclear industry where I worked as a project manager, to use my experience working in a safety critical industry and my personal experience of catastrophic healthcare harm, to influence safer healthcare. In 2023, I set up CareSafe Solutions as a way of offering freelance consultancy services and training courses to healthcare organisations and services.
